Lifest Health Care recognizes that NDIS participants often have a lot on their plates at either living with an intellectual, cognitive, neurological, sensory, physical, or psychosocial disability or having confidence in their ability to go outside their day-to-day lives. Within the NDIS plan that they develop with assisted funding, they can pursue their personal goals in a way they prefer, on their terms. In addition to managing their budget with assisted funding, NDIS participants can choose who will provide their support, how, where, and when. They can have funds managed by a third-party Plan Manager, the NDIA, or themselves, or a combination of management methods.
Our team of Ndis Social And Community Access in Sunshine Coast support coordinators is dedicated to assisting people with disabilities, their families, and caretakers by connecting them with NDIS facilities. We can assist people with disabilities in making their NDIS Community Participation plan a reality through various options, including personal development courses, social groups, and sporting clubs. For example, library visits, concerts, and movies are some of the things we can arrange for you.
Shopping Assistance – Have a support worker pick you up and help you shop. They will open your car door and provide a “royal experience.” Our staff will assist you in crossing the isles, loading the conveyor, and then safely depositing the trolley in your vehicle.
Events & Community Access – Our support workers can take you out for a day on the town and have lunch and a drink. We can help you access a day spa, beauty parlor, or nail or hair salon for pampering days.
Creating behaviour support plans – You desire to use behaviour support plans, so we can help you implement them. However, we prefer to develop them and assist you with the implementation to get the best results and meet your objectives.
Appointments – Have a support worker help you with your medical and disability appointments. They can drive you to your appointment, make sure you are on time, and take notes to give to your family, caretaker, etc.

Our Individualised Living Options are customisable support packages that can be interchanged to suit you and your unique needs. ILO includes services like support with daily living tasks, assistance in developing your skills, or assistance with household tasks.
Our Specialised Disability Accommodations, include a range of housing options that are designed specifically for our participants with extreme functional impairments or high support needs. The aim of our SDA is to make accessing support easier.
Our team bring to you our Shine House - located in Crestmead. At Shine House, we believe that life amounts to much more than just the serious stuff. It is sometimes as simple as coming together, finding a safe space as individuals. We have activities running Monday to Friday.
